Are you a highly experienced CHC Lead Officer / Team Manager ready to take ownership of a complex and high-profile portfolio? Reporting into the Service Manager for Hospital Social Work, you will be the Council’s lead expert on Continuing Healthcare, driving both operational delivery and strategic direction across Adult Social Care. This is a rare opportunity to step into a truly niche CHC Lead Officer / Team Manager role where your expertise will directly influence system-wide decision-making and outcomes.

Your role as a CHC Lead Officer / Team Manager:

  • Lead Continuing Healthcare (CHC) & FNC strategy, ensuring statutory compliance and best practice
  • Act as the principal liaison with NHS partners, ICBs and system leaders
  • Provide expert oversight on complex cases, DSTs and dispute resolution
  • Shape and implement policy, governance and quality assurance frameworks
  • Deliver impactful training and workforce development to build organisational capability

Your experience as a CHC Lead Officer / Team Manager:

  • Extensive hands-on experience in Continuing Healthcare within LA, NHS or ICB settings
  • Deep knowledge of the National CHC Framework and eligibility processes
  • Proven track record managing complex disputes and funding negotiations
  • Strong leadership credentials with experience influencing senior stakeholders and partnerships
  • Confidence in delivering training, mentoring and practice improvement
