Transitions Team Manager – Preparing for Adulthood Team (CWD)

12 Month Contract
£41 per hour
Inside IR35 – Via Umbrella
South West England
Hybrid – 3 days on site per week minimum

 
We’re looking for a dynamic Transitions Team Manager to lead our Preparing for Adulthood (PFA) Team within the 0–25 Disability Service. You’ll support children and young people with disabilities and special educational needs, helping them build independence and resilience at key life transitions.
This is a key leadership role during a time of service change, offering the chance to shape future support for young people.
 
 
Experience needed –

  • Qualified Social Worker (BA/MA/DipSW) registered with Social Work England
  • Strong experience in social care, including team management
  • Knowledge of legislation and best practice in children’s and adult services
  • Passion for enabling young people to thrive

 
Requirements –

  • Hybrid working: 3–5 days on-site per week
  • Enhanced DBS (child & adult) required
  • Travel: You’ll need access to a car and business insurance
