£280 per day (Inside IR35)

An excellent opportunity has arisen to provide exceptional support to young people with special educational needs and disabilities (SEND) as they transition to adulthood. We are now seeking a dedicated and experienced EHC Coordinator to join a busy London Local Authority.

Role Overview:

As an EHC Coordinator specialising in Preparation for Adulthood (PFA), you will play a crucial role in facilitating smooth transitions for young people with SEND. You will be responsible for coordinating and managing EHC plans, ensuring they are tailored to the individual needs of each young person, and focus on their aspirations for adulthood.

Key Responsibilities:

  • Manage and coordinate the EHC assessment and planning process for young people transitioning to adulthood.
  • Liaise with families, schools, and health professionals to gather necessary information for each EHC plan.
  • Ensure all EHC plans are compliant with statutory requirements and are reviewed and updated regularly.
  • Work closely with young people and their families to understand their goals and aspirations, and incorporate these into the EHC plans.
  • Provide guidance and support to young people, families, and professionals on PFA outcomes, including education, employment, independent living, and community participation.
  • Monitor and review the progress of each young person, making adjustments to the EHC plans as necessary.
  • Maintain accurate records and documentation in line with data protection and confidentiality policies.


  • Proven experience in EHC coordination or a similar role within SEND services.
  • Strong knowledge of the SEND Code of Practice and relevant legislation.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with young people, families, and professionals.
  • Strong organizational skills and attention to detail.
  • Ability to work independently and manage a caseload effectively.
  • Empathy, patience, and a genuine passion for supporting young people with SEND.
