Job Title: Assistant Head of Hospital Pathways
Contract Length: 6-month initial interim contract
Location: South West of England based local authority
Working arrangements: Hybrid
Pay rate: £400-£450 per day, full-time
IR35 Status: Inside IR35
 
Baltimore are working with a local authority client in the South West of England in their search for an experience hospital social care professional to support with strategic and operational delivery of the hospital discharge and assessment team, as the Assistant Head of Service.
Please note, for this role our client requires a Qualified Social Worker.

Your duties as Assistant Head of Service:

• Take lead of the management and development of the hospital discharge and assessment team, operating as a key member of the leadership team driving improvement and delivering high-quality services.
• Reporting to the Head of Service you will ensure an effective service delivery across the service, leading on policy and practice, quality assurance and performance.
• You will be leading on the hospital discharge and assessment teams, link in with partnership organisations across health in effective social care service delivery, ensuring service is operating within legislative requirements. 

 
Experience needed as Assistant Head of Service:

• Be a Qualified Social Worker with registered Social Worke England status.
• Experienced in leading social care teams within hospital settings.
• In-depth knowledge of social care legislation, quality assurance frameworks and performance management methods.
• Effective communication skills with the ability to work with providers and partnership organisations.
• Able to provide minimum two years worth of references.
