A rare opportunity in East Sussex County Council for an innovate leader to join our senior, strategic leadership team as a Head of Assessment and Care Management (Learning Disability & Transitions). Building on a foundation of success to deliver high quality services to the residents of East Sussex, you will not only drive the Learning Disability and Transitions functions through the Sussex Health and Care Partnership but also contribute to the success of the wider Adult Social Care directorate.

Transforming care is all about supporting the discharge of adults from acute locked hospitals and improving health and care services so that more people can live in the community, with the right support, and close to home. Part of Building the Right Support ESCC is part of the local Transforming Care Partnership. TCPs are made up of clinical commissioning groups, NHS England's specialised commissioners and local authorities.

Key Responsibilities

• Strategic and operational oversight of all Learning Disability and Transitions functions
• Overall responsibility for budget management of services (£2m staffing and £52m operational supporting over 45fte staff)
• Interface and representative of the Council to Health Partners and wider stakeholders
• Future, strategic and changeset management ensuring quality for money at every stage
• Performance management, coaching and mentoring
• Translating and implementing national and local policies into practice including the Care Act, Children and Families Act and Building the Right Support (Transforming Care Partnerships)

About you

• You will be an experienced leader with the foresight to innovate both operational functions and strategic service needs
• Strong background in operational management of Learning Disability services and integrated working functions. Transitions experience is desired but not essential.
• Astute knowledge of the Care Act, Continuing Health Care and good working knowledge of Transforming Care Programmes
• The ability to make decisions that focus on the needs of Service users even in the face of conflicting agendas between Health and Social Care
• Social Care qualification desired but not essential however you must demonstrate a high level of operational experience of Learning Disabilities & Transitions in a Local Authority setting.
